PROVIDE FOR THE SANITIZER SOLUTION TO BE MIXED TO THE APPROPRIATE LEVEL FOR THE TYPE OF SANITIZER BEING USED--FOR A CHLORINE SANITIZER, THE SOLUTION SHALL HAVE 50 PPM OF AVVAILABLE CHLORINE [750.830]. *35  OBSERVED THE SCREEN DOOR ATTACHED TO THE REAR EXIT DOOR TO BE IN POOR CONDITION. PROVIDE FOR ALL OPENINGS THAT LEAD TO THE OUTSIDE TO BE SELF-CLOSING AND TIGHT FITTING [750.1110]. 15 DAYS
OBSERVED NO LABELING INFORMATION ON ANY OF THE FOOD ITEMS WITHIN THE WALK-IN COOLER LOCATED IN THE MAIN KITCHEN. OBSERVED THE FOLLOWING POTENTIALLY HAZARDOUS FOODS TO BE LABELED WITH THE DATE AND TIME THAT THE COOLING PROCESS BEGAN [750.130]. 15 DAYS
OBSERVED THE FOLLOWING FOOD ITEMS BEING STORED ON THE FLOOR: (1) LARGE PLASTIC CONTAINER OF RAW CHICKEN WITHIN THE WALK-IN COOLER IN THE MAIN KITCHEN, (2) A BAG OF ONIONS LOCATED WITHIN THE WALK-IN COOLER NEAR THE PIZZA-MAKING STATION, AND (3) PLASTIC BUCKETS OF BEEF WITHIN THE WALK-IN FREEZER. PROVIDE FOR ALL FOOD PRODUCTS TO BE STORED AT LEAST SIX INCHES OFF OF THE FLOOR [750.130]. IN ADDITION, I OBSERVED NO COVER FOR THE BUCKET OF RAW CHICKEN WITHIN THE WALK-IN COOLER LOCATED AT THE MAIN KITCHEN. PROVIDE FOR ALL FOOD PRODUCTS IN STORAGE TO HAVE PROPER COVERS OR OVERHEAD PROTECTION [750.130]. IMMEDIATE/ONSITE

PROVIDE FOR ALL UTENSILS TO BE STORED IN ONE OF THE FOLLOWING MANNERS: (1) IN A CLEAN AND DRY CONTAINER , (2) IN THE FOOD PRODUCT WITH THE HANDLE UP AND OUT OF THE FOOD PRODUCT (3) IN A DIPPER WELL, USING POTABLE WATER, (4) MALT COLLARS AND FROZEN DESSERT UTENSILS EITHER CLEAN AND DRY OR IN A DIPPER WELL [750.280]. NEXT INSPECTION
15  OBSERVED BARE WOOD ATTACHED TO THE DOOR FRAME LOCATED NEXT TO THE PIZZA OVEN. PAINT AND SEAL THE BARE WOOD. PROVIDE FOR ALL FOOD CONTACT SURFACES TO BE SMOOTH, EASILY CLEANABLE, NON-ABSORBANT, CORROSIVE RESISTANT, AND LIGHT COLORED [750.690]. NEXT INSPECTION
